If you are coming to our office for your first visit, you may be unfamiliar with how we operate. To help you feel at ease from the start, we want to share information that you should know beforehand. Your First Visit
When you arrive for your first appointment with us, you’ll see your name on our welcome board. Before you meet with Dr. Jones and Dr. Amerson, we’ll take digital X-rays and all the necessary scans. Then, the two of you will discuss your goals and treatment options with a detailed exam. Our team will also go through all your flexible financing options. At that point, we can begin treatment. In fact, we can start that very same day if you want!
New Patient Forms
Although our waiting room is very comfortable, we understand that you probably want to spend as little time there as possible. For your first appointment, we will need to collect your insurance information, dental and medical history, and other information. You can help us process this this paperwork faster by completing these forms before you come to our office. That way, you’re ready to get started right away.
